Healthcare Subscriptions Terms & Conditions

 

Thank you for taking subscribing to our wellness package. Below outlines the terms and conditions:  

Your Direct Debit is collected by: Easy Direct Debits

  1. The cost, content and delivery of the goods and/or services paid for by this subscription is agreed between you and J. Horner Holdings Ltd trading as James Horner Vets.

  2. The wellness subscription only remains in force if you pay your monthly instalments, without default.

  3. The wellness subscription is an annual contract and will be automatically renewed by us. If you wish NOT to renew for a subsequent year, then you should notify us 21 days prior to your expiry date.

  4. You must be over 18 years of age.

  5. The wellness subscription is non-transferrable between pets.

  6. There is no insured benefit.

  7. Cancellation: The subscription may be cancelled at any time at your written request. Upon cancellation, you will be liable to settle the difference between the total cost of the goods and/or services received by you less the total amount collected by us. Any outstanding balances MUST be paid within 10 working days upon our request. If you have received benefits less than the value of the total paid, you will not be eligable for a refund.

  8. The subscription is designed to allow for monthly payment in advance for treatment administered at intervals, over a 12 month period. For a monthly fee James Horner Vets agree to provide the following goods and services (Delete as appropriate):

    • Gold Package

    • Silver Package

    • Bronze Package

  9. James Horner Vets may increase the monthly fee on 1st April in each year and will give two months’ notice in writing of such increase. James Horner Vets may increase their price list at any time. Any course of discounted treatment commenced before the increase and ongoing after the increase will be charged at the original discounted price. All other medication, treatment or additional services will be subject to normal annual price increases.

  10. James Horner Vets may terminate this Agreement by giving the Client not less than one month’s notice, provided that all treatments and services, paid for in advance, have been or will be supplied. The monthly fee must be paid until the termination of the Agreement.

  11. The Client may terminate this agreement by giving not less than one month’s notice, provided that all treatments and services have been paid for as intended over the twelve month period. Should the Client leave the subscription before this time, then the full value of the balance of payments to the end of the subscription must be paid within 10 working days upon request.

  12. In the event of the monthly charge being unpaid one month after it has become due, James Horner Vets may terminate this Agreement by giving notice to the Client to that effect. In the event, the Client will be liable for all monies then outstanding and due to James Horner Vets.

  13. In the event that the Client leaves the subscription within 12 months of receiving discounted treatment, or within 12 months from the end of a course of discounted treatment, James Horner Vets reserves the right to make a charge equal to the discount received by the Client.

  14. The monthly fee will be payable by Direct Debit to Easy Direct Debits who collect the fee on behalf of James Horner Vets. The Client will pay the fee monthly until this contract is terminated. The Client understands that Easy Direst Debits is only the administrating and collecting agent and therefore has no responsibility for standards of treatment or affairs of a clinical nature and disputes relating to treatment. Such affairs and disputes are strictly a matter between James Horner Vets and the Client.

  15. It is the Client’s responsibility to:

    1. Arrange with James Horner Vets for products and services to be delivered in time.

    2. Inform James Horner Vets of any change in circumstances or problems affecting the subscribed flock or farm.

  16. This Agreement is exclusive to the named subscribed pet belonging to the Client and James Horner Vets. The agreement is non-transferable between pets, including those owned by the same Client, and does not cover the services of any other practice.

  17. Any discounts offered as part of the subscription applies only to those pets and clients registered for the wellness package.

  18. Any dispute arising under this Agreement will be submitted to arbitration under the Arbitration Act 1996 at the joint cost of the parties.

  19. All notices given under the provisions of this Agreement must be in writing and sent to the last known address of the Client or James Horner Vets by recorded delivery post.

  20. Enrolment: All applications for membership will be effective on the date of the month as per the signed agreement.
